Please provide a copy of your current business license and resale tax permit.
      - If emailing, attach scanned image copies.
      - If mailing, include photocopies.

Necessary Legal Stuff:
Once we have received the above information, we will contact you and provide a copy of our Terms and Conditions form for you to review. We require that you return a signed copy of this form which will indicate that you have read and understand the information. When we have received your signed form, we will provide you details of our wholesale pricing and ordering information.
